YOUR CONSULTATION
An unhurried first
conversation.
Every member begins with a personal consultation with Marc Bloom, MD, the program’s Medical Director and a cardiothoracic surgeon. Here is what you will cover, how to prepare, and what you will leave with.

WHAT WE COVER
Nine conversations,
one complete picture.
Your consultation looks at your whole cardiometabolic picture, not a single number. The order follows your needs, and there is time for your questions throughout.
- 01
Your story
Your health history, any heart or vascular events or procedures, symptoms, and what prompted you to look into the program.
- 02
Your family history
Heart disease, stroke, diabetes, or high cholesterol in parents, brothers, or sisters, and the age it started.
- 03
Your medications
Every prescription, over-the-counter medicine, and supplement, including anything you have stopped because of side effects.
- 04
Your recent results
Bloodwork, ECGs, imaging, and reports from other doctors, so nothing is repeated without a reason.
- 05
How you live day to day
Activity, nutrition, sleep, stress, alcohol, and tobacco, and what has or has not worked for you before.
- 06
What matters to you
Your goals, from numbers you want to improve to the things you want to keep doing for years to come.
- 07
Your cardiometabolic picture
How your heart, blood sugar, weight, kidneys, and liver fit together, and where the biggest opportunities are.
- 08
Recommended next steps
Which included assessments are appropriate for you, what bloodwork to order, and your priorities across the four principles.
- 09
Your physician and the program
Whether your care continues with Dr. Bloom or with the cardiologist you choose or are matched with, and how membership works.
BEFORE YOUR CONSULTATION
Your preparation
checklist.
Tick items off as you go. Bring what you can; your coordinator can help with anything you cannot find.
0 of 13 ready
Your check marks are saved only on this device. Nothing is sent to the practice.
QUESTIONS YOU MAY WANT TO ASK
Write down
your own, too.
- Which of my numbers matter most, and what are my targets?
- Which of the included assessments are right for me, and why?
- Should my current medications change?
- What is the single most important change I can make first?
- Who will be my physician going forward, and how do I reach the team?
AFTER YOUR CONSULTATION
