IMPORTANT INFORMATION

Clear terms,
plainly stated.

What this website and the program do and do not promise. The membership agreement and the practice’s Notice of Privacy Practices contain the complete terms.

In plain English

  • This website teaches. It does not diagnose or treat you.
  • In an emergency, call 911.
  • Membership is not health insurance. Keep your insurance.
  • No one can promise results. Studies describe groups of people, not you.
  • Tests and medicines are used only when they are right for you.
  • Self-pay patients can ask for a written cost estimate before care.
  • Your health information is private and protected by law.

Medical information

The content on this website is general education. It is not medical advice, diagnosis, or treatment, and it does not replace an evaluation by a qualified clinician.

Reading this site, taking the Heart Check, or submitting an enrollment request does not create a physician-patient relationship. That relationship begins only after you enroll and are seen by a program physician.

Emergencies

The program is not an emergency service. If you have chest pain, severe shortness of breath, fainting, signs of a stroke, or any other emergency, call 911. In a mental-health crisis, call or text 988.

No guarantee of results

Medicine is not an exact science. The program cannot promise any result. This includes changes in weight, blood pressure, cholesterol, blood sugar, fitness, heart risk, quality of life, or how long you live. Results depend on many factors, including your health history and participation.

About the research we cite

Studies cited on this site describe groups of people, not individuals. Studies of large groups show links, not proof of what will happen to any one person. Clinical trials tested specific treatments in specific groups of people. Your physician will discuss how the evidence applies to you.

We cite studies and organizations such as the World Health Organization, the American Heart Association, and the American College of Cardiology. That does not mean they endorse the program.

Membership is not insurance

Membership is not health insurance. It does not qualify as minimum essential coverage under the Affordable Care Act and is not workers’ compensation insurance. Please keep health insurance for hospital care, emergencies, medications, and services outside the program.

The program is cash-pay. The practice does not submit claims to health insurance for the membership, visits, or included assessments. Bloodwork may be billed to your insurance by the laboratory. If you have Medicare or another government health plan, please tell the coordinator before you enroll. Federal rules may affect whether you can join.

The membership agreement explains the length of membership, fees, cancellation rights, and refund policy. You receive it and can review it before you sign or pay.

Assessments and treatment

Included assessments are performed only when clinically appropriate for you, as determined by your physician. Inclusion in membership is not a recommendation that every member have every test. Any test can reveal unexpected findings that need further evaluation at additional cost.

Medicines, including weight-loss medicines such as GLP-1s, are prescribed only after your doctor evaluates you, and only if they are right for you. Not every patient is a candidate. Medication costs are not included in membership.

Telehealth

Video visits are offered when they are medically appropriate. During the visit, you must be in Florida or another state where your doctor is licensed. Some care can only be provided in person.

The Heart Check

The 2-Minute Heart Check is an educational tool. It is not a diagnosis, a risk calculation, or a substitute for medical evaluation. Your answers are processed only in your browser and are not stored or sent to the practice.

Photographs and sample reports

Photographs and illustrations on this site are illustrative. Some are digitally created and show fictional people; none show actual patients or members. The sample Heart Report and sample Personal Cardiometabolic Plan describe a fictional patient with fictitious values. They are for illustration only, do not describe any real person, and do not predict your results.

Privacy and this website

This website does not collect medical information. The enrollment request form collects contact details only, used to respond to your request. Please do not send medical details through the form, regular email, or text message.

We do not use advertising trackers on this site. Links to other websites are provided for convenience; we are not responsible for their content or privacy practices. The patient portal and online payments run on the practice’s secure health record system (eClinicalWorks / healow). The practice’s Notice of Privacy Practices covers them.

Patient health information is protected under HIPAA and described in the practice’s Notice of Privacy Practices, which is also provided at enrollment.

About the program

The Comprehensive Cardiovascular Optimization Program is offered by Tampa Cardiovascular Associates, 4450 E. Fletcher Avenue, Suite C, Tampa, FL 33613. Physician credentials are listed as provided by the practice. The program serves adults aged 18 and older and does not discriminate on the basis of any characteristic protected by law.

Your right to a Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance, or who are not using insurance, an estimate of the bill for medical items and services. Because the program is self-pay, this applies to you.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059. To request an estimate from us, call (813) 975-2800.
