FOUNDING MEMBERSHIP
Becoming
a member.
A personal process from your first request to your first comprehensive visit, with a coordinator guiding each step and time to ask questions before you commit.

HOW ENROLLMENT WORKS
Six steps.
No pressure at any of them.
You will speak with a person, meet Dr. Bloom, and read every term before any payment is made.
- 01
Request enrollment
Call our coordinator at (813) 975-2800. The call takes a few minutes, and we will not ask for medical details yet.Complete the short form on this page. It takes about two minutes and asks for no medical information.
- 02
A call from your coordinator
A program coordinator calls to answer your questions, confirm the program fits your needs, and schedule your consultation.
Already a Tampa Cardiovascular Associates patient? Your records are already here, and your transition into the program is coordinated with your current cardiologist.
- 03
Your consultation with Dr. Bloom
Every member begins with a personal consultation with Marc Bloom, MD, the program’s Medical Director, to discuss your history and goals and confirm the program is right for you.
Afterward, your care may continue with Dr. Bloom, or you can choose, or be matched with, one of the program’s cardiologists based on your needs.
What the consultation covers - 04
Review and sign your agreement
You receive the membership agreement and consent forms electronically to read at your own pace. Your coordinator will walk you through the terms and answer any questions before you sign.
- 05
Your annual payment
The $1,000 membership fee is paid in a single annual payment. Visits are $100 each, paid at the time of the visit.
- 06
Your first comprehensive visit
An unhurried review of your history, medications, recent results, and goals. Appropriate assessments are scheduled, and you leave with a personal plan across the program’s four principles.
MEMBERSHIP AT A GLANCE
Clear terms,
before you commit.
A summary of the key terms. Your membership agreement contains the complete terms, and you will have time to review it before signing.
- Membership term
- Twelve months from your membership start date. Renewal is offered before your term ends; nothing renews automatically.
- Annual fee
- $1,000, paid in one payment. The fee covers your membership and the included assessments when they are appropriate for you.
- Visits
- $100 per visit, paid at the time of each visit. Follow-up visits may be by secure video through healow when appropriate.
- Included assessments
- Echocardiogram, carotid ultrasound, exercise stress test, and abdominal aortic aneurysm screening, when clinically appropriate. See each one explained
- Yearly Heart Report
- A written summary of your key numbers, progress since joining, and priorities for the year ahead.
- Bloodwork and other tests
- Bloodwork may be billed to your health insurance. Any other recommended test is explained, and its cost discussed, before it is ordered.
- Cancellation
- Your annual fee reserves your cardiologist’s time and funds your included assessments. After a 14-day review period, it is non-refundable except in specific circumstances such as relocation, a serious medical condition, or if the practice can no longer provide the program. Either party may end the agreement with 30 days’ written notice.
- Your physician
- You begin with a consultation with Dr. Marc Bloom. Your ongoing care is then led by Dr. Bloom or by the program cardiologist you choose or are matched with.
- Privacy
- Your records are protected under HIPAA and shared only with those involved in your care or with your written authorization. Privacy and discretion
- Cost estimates
- As a self-pay patient, you have the right to a written Good Faith Estimate of expected charges before care. Learn about your rights
- Health insurance
- Membership is not health insurance. Please keep your current coverage for hospital care, emergencies, medications, and services outside the program.
