HOW THE PROGRAM WORKS
Who the program is for, what each principle covers, and what membership looks like from your first visit to your yearly Heart Report.

WHO THE PROGRAM IS FOR
Cardiovascular risk can persist even with regular care. The program looks at your medical risks, how you move, your weight, and your well-being together, in one plan.
For adults who have had a heart attack, a stent, bypass surgery, or a stroke, or who have narrowed arteries in the heart, neck, legs, or the aorta.
Cholesterol or blood pressure that is hard to control. Trouble taking statins. A high Lp(a) or another inherited cholesterol problem. Or a close relative who had heart disease young.
Prediabetes or diabetes, insulin resistance, extra weight around the waist, high triglycerides, fatty liver, or early kidney disease. Together they speed up heart disease, and they respond best when treated together.
Why it mattersFor people who want help exercising safely, losing weight in a lasting way, and handling the stress that can come with a heart diagnosis.
Follow-up, shared decisions, and lasting change are central to the approach.
Four principles. One coordinated plan.
Cardiovascular health and quality of life are the focus.
OUR FOUR PRINCIPLES
A framework designed by heart doctors. It connects your medical care with your daily habits and support. It draws on World Health Organization (WHO) guidance.
Every plan starts with your health history, what you can do today, and your goals. Your doctor shapes it to fit you.
Explore the WHO guidanceWe review your cholesterol, blood pressure, and any heart conditions together. Treatment choices are based on your overall risk.
An exercise plan built for you, based on your heart history, your fitness, and any limits you have.
Weight loss and long-term weight management, when appropriate, as part of a broader cardiometabolic plan. Reducing fat around the organs can improve blood sugar, blood pressure, and cholesterol together.
Benefits and risks of any treatment are reviewed with you individually. Medication costs and coverage are confirmed before treatment begins.
Mental health is a core part of the program. Your care includes attention to how you are coping, with connection to appropriate support.
Therapy and psychiatric care from outside providers are not included in membership. They are arranged separately.
THE GUIDANCE BEHIND THE PROGRAM
The program draws on WHO advice on heart risk, exercise, weight, and mental health. The four principles are the program’s own framework; WHO does not endorse the program or its membership.
THE MEMBER EXPERIENCE
Understand the risk. Choose the priorities. Measure progress over time.
We look closely at your heart and family history, your medicines, your activity, your weight goals, and how you are feeling. We also decide which included tests suit you.
Together we choose what matters most first: lowering your risks, moving more, losing weight if needed, and your well-being.
Review response, medication tolerability, and practical barriers. Treatment and support adapt as your needs change.
Connect progress with what matters to you: stamina, strength, independence, confidence with activity, and time with family.
YOUR YEARLY HEART REPORT
Each year you receive a written Heart Report, organized around the four connected systems the program treats. It shows your key numbers, how they have changed since you joined, and your priorities for the year ahead. Your doctor goes over it with you.
Regular follow-up is built into the program, and we work with the doctors you already see. You will know how often you visit and how to reach us before you join.
With your permission, your spouse, family, or personal assistant can be kept in the loop.
PHYSICIAN-LED, COORDINATED CARE
Dr. Marc Bloom, a cardiothoracic surgeon, leads the program as Medical Director. He is joined by the cardiologists of Tampa Cardiovascular Associates. Every member begins with a consultation with Dr. Bloom. After that, your care continues with him or with the cardiologist you choose or are matched with.
